Life expectancy is improving in Asia-Pacific, infant mortality falling and health spending rising, says OECD.

The OECD’s new Health at a Glance: Asia/Pacific 2010 says that Asian economies spend just over USD 500 per person per year on health, against over USD 3000 in OECD countries. This amounts to more than 4% of GDP, on average, compared to over 9% in OECD countries.

The report presents key indicators on health and health systems for 27 Asia/Pacific countries and economies, including four OECD member countries (Australia, Japan, New Zealand and the Republic of Korea).

The current recession affecting most of Europe has reduced the inflow of immigrants. Many foreign workers are returning to their home countries as job opportunities decline and unemployment rises. Those who stay face a reduction of social benefits and in same cases discrimination.

Secretary-General Ban Ki-moon on 1 October 2010 urged governments to institute measures to provide greater support to a growing number of older people, saying they played an important role in society as leaders, caregivers and volunteers, yet they faced discrimination, neglect and abuse.

‘The key interventions are well-known: granting universal access to social services; increasing the number and worth of pension plans; and creating laws and policies that prevent age and gender discrimination in the workplace,’ Mr. Ban said in a message to mark the International Day of Older Persons, the 20th year that the day is being observed.

要するに、先進国といわれている国が、それなりの社会保障として高齢者の介護・支援を行う社会システムを作ってきたのですが、自国の看護・介護の専門職だけで担うことができず（看護・介護労働力不足で）、かなりの部分を外国人労働者に依存しているという実態です。

　たとえば、スエーデン。「外国人労働者、大いに歓迎です！　違う価値観が共存することが、国を豊かにします」。

イギリスは先日厚生労働省の幹部がすこやか福祉会の認知症のグループホームに視察にいらっしゃいましたが、「認知症の人のケアについてはイギリスより日本のほうが進んでいます。イギリスの介護労働は他の国の人をあてにしています。たとえば、ケースワーカーでさえ半数以上が外国人です」。

フランス・・・「家庭内の支援は、北西アフリカのモロッコなどの、もと植民地化していた国々の労働者に依存しています」。　

ドイツ。「トルコなどの国からの移民問題がずっと続いている。それと介護問題が重なり合っている」

　いわゆる先進国は、高齢社会・介護問題でそれなりのシステムを作ってきたが、それを担う人・労働者・専門職がいなくて苦心しているのです。それを専門職としての介護労働者という位置づけではなく、差別的なニュアンスをもった位置づけでシステムに組み込んでいるように見えるのです。

カナダ・オーストラリアだって、発展途上国といわれている国からの介護労働者を膨大な数を受け入れることにエネルギーを裂いています。

都老研講演会

在宅サービスの定義やサービス内容は国ごとに差異がありますが、各国に共通しているのは、訪問看護サービスと家事援助・介護サービスです。これらは、６つのサービス群に分けられます。

　第一は、核となるサービスで、身体、精神、認知の機能に対する診断と評価を行い、ケア・プランを作成し必要なサービスの調整をはかります。

　第二は医療・介護サポートで、看護や医療のケアがこれに含まれます。

　第三が家事介護サポートで、清掃、食事、洗濯などの日常家事から、高齢者本人の入浴、食事、排泄、散歩等の生活機能の補助があります。

　第四が住環境サポートとして、家の保持・管理などです。

　第五が法的・財政的サポートで家計管理・財産管理・法務処理などです。

　最後がソーシャル・サポートとして、訪問、話し相手になるなど、障害の有無に関係なく、生き生きとした生活を送るために必要なサービスです。
